Kevin Feige is once again proven to have blown it and obviously doesn’t have a clue in the world when it comes to the comics.
Off the top of my head, it’s known that Feige passed on comic-accurate versions of Skaar as well as MODOK (see below). The female Silver Surfer also looks horrendous.
Comic accurate look dropped
The original design for Tim Blake Nelson as The Leader has now been released online for Captain America: Brave New World, which is a comic book-accurate take on the character.
The VFX company behind the design even says they were inspired by the comics.
However, the company says the creative direction changed and their version was ditched:
We were originally brought on to design and apply the practical makeup for The Leader on the incomparable Tim Blake Nelson in Captain America: Brave New World. As often happens in film, the story evolved, and during reshoots, the creative direction shifted. Our version was ultimately not used in the final cut.
Still, we remain incredibly proud of the work we created.
Drawing inspiration from the original comic book art, we developed a look that felt both grounded and iconic—true to the source while achieving a refined, natural realism. Technically, we were thrilled with how lightweight and comfortable the final makeup was for the actor—an achievement made possible by the dedication of an extraordinary team.

New look at The Leader in Captain America: Brave New World
A new image of Tim Blake Nelson as The Leader has also been released via the art book for Captain America: Brave New World.
I know a lot of fans don’t like it. The above version also does look a lot better and fits more with the MCU version of the character.

Regarding Skaar, the son of Hulk, Kevin Feige opted to go with this goofy man bun version instead of the badass look from the comics, which was created in concept art:

For MODOK, Feige thought it was better again to go the goofy route instead of the more comic book accurate take, which in the comics is generally more horrifying:
Silver Surfer in Fantastic Four vs 2007 FF:
Feige also gender-swapped the Silver Surfer in the upcoming Fantastic Four movie. The Silver Surfer looked better 20 years ago.
